In order to better serve your needs, as of January 2013 we have instituted a Billing Team that will be able to assist you in any way with insurance claims, flexible spending accounts, and payment options.  We can be reached via email at billingdept@amylight.com

As of January 1, 2015 we will no longer be participating providers with United Concordia. 

We participate with DELTA DENTAL and METLIFE Insurance Companies.  Per these insurance carriers, we will be collecting 20% of all services rendered at the time of each visit.  

For members with insurance carriers other than Delta Dental or MetLife, as a courtesy, we will electronically submit your insurance claim if a copy of the insurance card and all required insurance information is provided. This will allow faster reimbursement to you. 

Payment in full is due at the time services are rendered.

The office accepts personal checks, cash, Visa, MasterCard, Discover, American Express, CareCredit and Spring Stone Financing.
